Physicians from Acibadem City Clinic Tokuda Constructed and Implanted a New Jawbone to a Patient with Tumour

A team of maxilla-facial surgeons, orthopedists and vascular surgeons performed a complex multidisciplinary operation with an autologous graft from shinbone

An avant-garde reconstructive operation in the maxilla-facial area performed in Acibadem City Clinic Tokuda Hospital will make it possible for a woman with a mandible tumour to eat and have a normal life. A team of maxilla-facial surgeon Assoc. Prof. Anton Dzhorov, Assoc. Prof. Vasil Yablanski, the head of Orthopaedics and Traumatology Department, and prof. Vasil Chervenkov, the head of Vascular Surgery Department, performed a complicated surgical intervention when the patient’s mandible was replaced with a tibia. The operation lasted 9 hours.

The patient was diagnosed with a mandibular ameloblastoma after having detected a suspicious area when an x-ray was made at the extraction of a wisdom tooth. The tumour was considered to be aggressive, it could spread quickly and affect the adjoining tissues and organs and often turns into a malignant formation. For an efficient treatment 1/3 of the mandibular volume should have been removed. So, it must have been replaced so that the patient could eat and have a decent look.

The operation proceeded at a couple of stages. First, Assoc. Prof. Dzhorov with the help of Dr. Veselin Nikolov and Dr. Doychin Dochev removed the mandibular part affected by the tumour while orthopedists team of Assoc. Prof. Yablanski and Dr. Evgeni Vlaev parallely took autologous transplant – part of patient’s fibula together with the respective artery and vein (fibular auto-transplant). The implant was treated, anatomically shaped and fixed after that a team of vascular surgeons – prof. Chervenkov and Dr. Nikolay Valchev performed microsurgical bonding of its feeding vessels with the vessels in the mandibular area.

The operation took place due to the excellent collaboration with colleagues surgeons in different specialties in the hospital and therefore we would like to thank them. The patient is feeling and recovering well and soon she is going to be discharged, shared Assoc. Prof. Dzhorov.
